The Genes & Lifestyle Reality: Demystifying APOE4
Mention genetics, and most people assume it’s a done deal.
Hearing "I have the gene for..." can feel like living with a ticking time bomb—a matter of time before symptoms inevitably show up. But when it comes to neurodegenerative markers like APOE4 (often dubbed the "dementia gene"), that is simply not how genetics work.
Carrying the APOE4 allele does not mean cognitive decline is your guaranteed fate (Livingston et al., 2020). Host genes can sit dormant our entire lives, waiting to be triggered—or never expressed at all. While some of it comes down to luck, a massive portion lies directly within our control.
Feed the Soil, Not the Plant
If you're a gardener, you already understand the golden rule: feed the soil, not the plant. Healthy soil is the foundation of a thriving garden.
Your body operates on the exact same principle. In this case, your "soil" is your biological terrain: your cardiovascular health, metabolic efficiency, and nervous system integrity. Look after the terrain, and your energy, mental clarity, and focus will flourish.
The secret to keeping our marbles intact for as long as possible is to start looking after our brain early. And by early, I mean whatever age you are right now:
In your 20s, 30s or 40s? Brilliant. Let's build your cognitive reserve.
In your 50s, 60s, or 70s? Yes, yes, yes! It’s never too late to shift the dial—though the longer your run-up, the better your results. Don't wait for the first cognitive slip to start making moves.
The Big Drivers of Brain Terrain
If we want to keep APOE4 and cellular damage in check, we need to address the key factors that inflame and age brain tissue:
Oxidative Stress (Smoking, Vaping & Alcohol): These introduce massive free-radical damage to delicate brain tissue. Stopping smoking is a non-negotiable for brain longevity.
Sleep Deprivation: Quality sleep is your brain's only window to run its glymphatic system, the internal waste-clearing process that flushes out cellular debris while you rest (Jia, Liyun et al., 2025).
Insulin Resistance & Blood Sugar: Poor metabolic health starves brain cells of steady fuel, directly accelerating cognitive decline (Majid,, Haya et al. 2025). It’s a big issue that I wrote a lot more about here!
High Blood Pressure: Chronic arterial pressure weakens the delicate blood vessels in your brain, restricting oxygen supply and increasing vascular risk.
Simple, High-Impact Micro-Habits
It sounds like a lot, but because everything in the body is connected through metabolic health, small daily tweaks create a massive ripple effect:
Walk After Meals: A gentle 10-minute walk lowers post-meal blood sugar spikes, supports healthy blood pressure, and boosts cerebral blood flow.
Prioritize Restorative Sleep: Identify your specific sleep blocks. Is it perimenopausal night sweats, lingering stress, or underlying sleep apnea? Better sleep calms systemic inflammation, stabilizes appetite hormones, and reduces late-night sugar cravings.
Rethink the Nightcap: Alcohol might act as a temporary social lubricant, but it disrupts deep sleep, stresses the liver and pancreas, and directly inflames brain cells. Moderation is key. When your baseline health is solid, your body copes with the occasional drink much better.
Eat for Brain Resilience: Load up on rich anti-inflammatory fats like extra virgin olive oil, oily fish, and flaxseeds. Paired with colorful, antioxidant-rich plants and quality protein to keep blood sugar rock-solid.
Reclaim Your Metabolic Baseline
If you feel overwhelmed trying to pull all these pieces together on your own, you don't have to do it in isolation.
In clinic, I guide clients through a structured metabolic framework designed to kickstart cellular metabolism. By using food to balance blood sugar, lower blood pressure, and reduce systemic inflammation, we create the ultimate protective terrain for your brain and nervous system.
